Notification of Serious and Significant Incidents

Ensure that major and serious incidents that occur across Qualico Regions and Business Units are reported promptly to the Qualico Regional Vice President (RVP). This is required so that:

Senior leadership and Company owners are promptly informed of such incidents and,

Appropriate media or other strategies and responses can be implemented in a timely manner.

Note: This requirement does not replace or alter any incident reporting and investigation requirements that may be in place as a result of Applicable Legislation and Qualico’s Health & Safety Program.

Responsibility

Regional Safety Managers are responsible for reporting all incidents on the day of the occurrence. Major or serious incidences should be reported to the Regional Vice President. The Regional Vice President is responsible to ensure the report is reviewed and forwarded to the required parties.

Application

Notification requirements apply to all major and serious incidents that occur on all Qualico Work Sites.

Method of Notification

The Regional Safety Manager shall use the Serious Incident Reporting form.

What Major and Serious Incidents to Report
  • Any incident that is reportable under Applicable Legislation, (Manitoba – Serious Incidents, Saskatchewan – Investigation of Certain Accidents, Alberta – Serious Injuries and Accidents, British Columbia – Accident Reporting and Investigation, Texas – OSHA and TDI-DWC).
  • Any other major incident or occurrence that might occur near or adjacent to a Qualico Work Site.
  • Any major incident (occupation, non-occupational or undetermined) that:
    • May expose Qualico to media exposure.
    • Involve or will likely involve police, any other law enforcement or regulatory agency involvement (other than routine or scheduled inspections).
    • May have the potential to impact upon the reputation or image of the company.
    • You believe, based upon your experience and knowledge, that you feel senior leadership should be made aware of.
    • May receive notices, stop work orders or fines from the Regulatory Bodies.
    • If in doubt, report the incident.
